Passport Information

THE TOWN OF WEST NEWBURY DOES NOT CURRENTLY ISSUE PASSPORTS. PASSPORTS ARE ISSUED BY THE U.S. SECRETARY OF STATE, PASSPORT DIVISION

SUMMARY OF INFORMATION YOU WILL NEED TO APPLY FOR A PASSPORT:

IF YOU WANT TO APPLY FOR YOUR FIRST PASSPORT OR IF YOUR PASSPORT IS OVER 15 YEARS OLD:

1.  COMPLETED APPLICATION FORM DS-11
2.  IDENTIFICATION: Acceptable documentary evidence of identity (e.g. Driver's License)
3.  TWO (2) PASSPORT PHOTOS: Identical, 2"x 2", full face, plain light background.
4.  BIRTH CERTIFICATE: Original or certified copy, must contain the following info:
a.  Name of Applicant
b.  Date of Birth
c.  Place of Birth
d.  File or Registration Date
e.  Certification (raised or colored seal)
(it is HIGHLY recommended that you submit a "long form" copy complete with parental information; extract cards are often insufficient and cause delay)

If you were born in WEST NEWBURY (or your parents were living in WEST NEWBURY when you were born):

1.  Mail a letter with your name (at birth) and date of birth to:

2.  Include a check for $10.00 per birth certificate (payable to Town of West Newbury)

3.  And a self-addressed stamped envelope.

If you were born in the U.S (outside of West Newbury) and need a birth certificate:

Contact the Town Clerk or City Clerk in the municipality in which you were born, or place a request for your birth certificate to the State Vital Records Division of the Massachusetts Department of Public Health

If you are a U.S. citizen who was born outside of the U.S. and need a Consular Report of Birth Abroad:

RENEWAL BY MAIL IF YOU HOLD A CURRENT VALID PASSPORT OR RECENTLY EXPIRED PASSPORT:

You are qualified for this if your old passport was issued:

A. Within the last 15 years
B. After your 16th Birthday
C. You can submit your most recent passport
D. You are using the same name or can submit the marriage certificate or court order showing the change of name

If you fulfill all of the above, submit an application for Passport by Mail (form DS-82), along with two new photos, your old passport, and a $55.00 check or money order payable to the U.S. Dept. of State to the address on the back of the application form.

NOTE: Children's renewals must be done in person with form DS-11, Both parents should appear, along with the child, and the child's birth certificate is required (even if you have the child's passport) in order to prove parentage.
